Request Alternate Media
Please use the links below to submit a request for Alternate Media. You should submit your request as soon as you have decided to purchase your book(s) or utilize your accommodation(s) for the semester.
Ideally, ASC should receive requests before the start of the Semester to ensure that you receive materials in a timely fashion. The faster you submit a request, the sooner you will receive your material. Please click the appropriate link below to complete your request:

Textbooks can be requested in alternative formats, including audio books, digital text files, and Braille. Textbook requests require that a student provide proof of purchase to ASC staff. - Note Taking Accommodations

Note taking accommodations include smart pens, audio note takers, and captioning. These products typically include mandatory training by the Alternate Media Specialist. Students placing these requests will be contacted via phone or email for further instruction. You do not need to fill out a form if you have used note taking accommodations in the past. - Speech-To-Text Dictation

Students can use dictation software on campus computers, personal computers, and mobile devices. These products typically include mandatory training by the Alternate Media Specialist. Students placing these requests will be contacted via phone or email for further instruction. You do not need to fill out a form if you have used dictation accommodations in the past.
Requests will be reviewed based on the student’s Educational Accommodations form, provided and approved by ASC Counselors. If you are new to ASC and need to schedule an appointment with a counselor, please contact the ASC front office at (707) 864-7136.
